Qualifications

• Minimum of three years nursing experience, with one year hospice experience.
• Ability to physically access any home and manage the stress of working with terminally ill patients.
• Experience working with an EMR/EHR system is preferred.

Responsibilities

• Oversee intake staff for accurate and timely processing of referrals.
• Assist scheduling staff with escalations from the Care Navigation Center.
• Maintain relationships with referral sources and internal teams.
