Transitions Hospice in Illinois is seeking an RN Case Manager to coordinate care and lead the interdisciplinary team, ensuring 24/7 support for home-based patients. You will serve as the clinical anchor, conducting assessments, guiding care plans, and communicating with families and physicians.

The role requires an active RN license in the state, nursing degree, and hospice/home health experience. Expect on-call duties and a compassionate approach to end-of-life care.

Competenze

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• 1+ year Hospice/Home health experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication skills across care teams and families.

Mansioni

• Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
