Job summary
Holisticare Hospice is seeking a Full Time Registered Nurse Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.
You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey in Monmouth area. Schedule is Mon-Fri, 8am-4:30pm.

Qualifications
- Must hold current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus.
- Proficient in typing and EMR software; CPR as required by state.
- Valid driver's license and reliable transportation.
Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner with Clinical Director to assess eligibility and suitability for hospice services.
- Nurture direct care: coordinate care while ensuring patient comfort and quality of life.
- Deliver nursing care rooted in proven principles and techniques to ease pain and provide solace.
- Educate and advocate for patients and families to create personalized care plans.
- Participate in call duties as part of the team.
