Job summary

Holisticare Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our nursing team delivering compassionate end-of-life care across the Monmouth area. You will coordinate services, support patients and families, and collaborate with an interdisciplinary team in a role that blends clinical excellence with connection.

Schedule: Monday - Friday, 8 am - 4:30 pm. Requirements include RN licensure, hospice or acute care experience preferred, EMR proficiency, and driving to patient

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
