Job summary

Sanctuary Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our compassionate team delivering end-of-life care across the community. You will coordinate services, support patients and families, and collaborate with an interdisciplina ry care team to ensure comfort and dignity.

This role covers Hoffman Estates, Barrington, Cary, Crystal Lake, Streamwood and Algonquin (Lake & McHenry Counties).

Qualifications

• Hold current unencumbered RN license.
• Minimum one (1) year RN experience in medical surgical/acute care; hospice a plus.
• Must be computer proficient with EMR experience.
• Current CPR certification if required by state.
• Valid driver’s license, vehicle insurance and reliable transportation.
