Job summary

Crescent Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to deliver hands-on hospice nursing care and coordinate services for patients in the Augusta area.

Join our mission-driven team for compassionate end-of-life care, with a $5,000 sign-on bonus and comprehensive benefits. Expect schedule Monday–Friday, 8 am–4:30 pm, with strong onboarding and ongoing professional development.

Qualifications

• Must hold current unencumbered Registered Nurse license.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in typing and computer programs; EMR background preferred.
• CPR certification as required by state.
• Must have a valid driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess eligibility and plan care.
• Deliver direct patient care with focus on comfort and quality of life.
• Educate patients and families to participate in personalized care plans.
• Participate in call duties as part of the team.
• Offer compassionate support to patients and families throughout hospice journey.
