Job summary

Optum is seeking a Registered Nurse Case Manager for hospice care. You will assume full nursing responsibility for the Plan of Care, collaborating with professionals to deliver holistic care addressing physical, psychosocial, emotional, and spiritual well-being. Experience with end-of-life care is preferred.

Position available as Full-Time and PRN. Sign-on bonus of $5000 for external full-time candidates. Competitive salary with comprehensive benefits package.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Make initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs; re-assess at least every 15 days.
