Job summary

Holisticare Hospice in Red Bank, NJ, is seeking a Full Time Hospice RN Case Manager (RNCM) to deliver hands-on end-of-life nursing care, coordinate services, and support patients and families through the hospice journey.

This role offers a Monday–Friday schedule, a structured onboarding program, and robust benefits including health insurance, PTO, tuition reimbursement, and a company 401(k) plan.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum 1 year experience as an RN in a medical-surgical/acute care setting; hospice experience a plus.
• Must be computer proficient in typing and EMR.
• Maintain CPR certification if required by state.
• Current driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Respond to clinical referrals with compassionate care and timely inquiries.
• Collaborate with Clinical Director to assess eligibility and suitability for hospice services.
• Provide direct nursing care to ensure patient comfort and quality of life.
• Deliver nursing care rooted in proven practices to ease pain and distress.
• Educate patients and families to participate in creating personalized care plans.
• Share on-call duties as part of the team.
