Job summary
Crescent Hospice is hiring a Full Time Hospice RN Case Manager (RNCM) to deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey in the Augusta area.
We offer a sign-on bonus and a comprehensive benefits package, plus opportunities for growth within a mission-driven, patient-centered organization. Schedule is Monday–Friday, 8 am–4:30 pm with a supportive onboarding program.
Qualifications
- Active, unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
- Proficient with computers and EMR systems.
- Current CPR certification if required by state.
- Valid driver's license and reliable transportation.
Responsibilities
- Respond to clinical referrals with care and expertise.
- Collaborate with Clinical Director to assess client eligibility for services.
- Provide direct nursing care to ensure comfort and quality of life.
- Deliver nursing care rooted in proven principles and techniques.
- Educate and advocate to involve patients and families in care plans.
- Share call duties as part of the interdisciplinary team.
