Job summary

Carolinas Medical Center seeks a Registered Nurse Case Manager to join the Oshkosh Hospice team. Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m., with rotating holidays. External candidates may receive a $3,000 sign-on bonus. The role focuses on compassionate, patient-centered end-of-life care across hospice settings in Oshkosh.

Ideal candidates are collaborative and committed to high-quality care, coordinating patient and family needs, and leading IDG care planning in a dynamic hospice environment.

Qualifications

• Registered Nurse license issued by the state where practice occurs.
• Basic Life Support (BLS) certification required upon hire unless exempt.
• Valid driver’s license issued by the appropriate Division.

Responsibilities

• Identify and coordinate patient and family care to support terminally ill patients and families across hospice settings.
• Educate patients and families about disease processes, hospice philosophy, and end-of-life care.
• Monitor and optimize utilization of services to meet hospice level-of-care criteria.
• Administer medications and treatments as prescribed by the physician.
• Lead the interdisciplinary care group and develop the patient’s Plan of Care with the IDG.
• Document in the Electronic Health Record per agency standards and policies.
• Provide ongoing assessment of patient response and update the plan of care as needed.
• Supervise Home Health Aide and LPN visits according to regulations.
