Job summary

Caretenders Hospice, part of the Optum family, is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients, collaborating with the IDG and physicians to promote physical, psychosocial, emotional, and spiritual well-being.

This full-time role includes a $5,000 sign-on bonus and comprehensive benefits. You will perform initial evaluations within 48 hours, plan and coordinate care, and support patients and families in daily living and comfort.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.
• Preferred: 1+ years of clinical experience; CPR certification or ability to obtain within 90 days.

Responsibilities

• Perform initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Assess patient/family needs and re-assess at least every 15 days, updating the plan of care.
• Initiate and coordinate the plan of care and document goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG members to provide daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate and timely clinical notes on patient condition.
