Job summary

Allay Hospice in Wisconsin is seeking a Registered Nurse Case Manager to deliver patient-centered hospice nursing care in home and community settings. You will coordinate care with an interdisciplinary team to ensure comfort, dignity, and goals are met for patients and families.

The role offers a $5,000 sign-on bonus and a collaborative environment with strong support from social work, chaplaincy, Aides and providers. Schedule is full-time with travel within the territory as needed.

Qualifications

• Graduate of an accredited nursing program with current state licensure.
• Minimum of 1 year nursing experience; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care and compassionate patient-centered approach.
• Strong commitment to clinical and documentation excellence.
• Must have a valid driver's license and reliable vehicle.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans aligned with patient wishes and goals
• Direct nursing care: administer medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patients' families and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
