Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to deliver personalized end-of-life care in patients' homes and other care settings. You will assess needs, implement care plans, educate families, and collaborate with an interdisciplinary team to ensure high-quality, patient-centered care.

Qualifications

• Current nursing license in the state of employment.
• CPR certification required.
• Current automobile insurance and valid driver’s license.
• Minimum 1 year RN experience; hospice or palliative care preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person or telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with visit frequency and risk identification.
• Coordinate care with IDG and physicians; support transitions in levels of care.
