Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to deliver compassionate, patient‑centered care in patients' homes across Texas. You will perform comprehensive assessments, develop and adjust plans of care, and educate patients and families on symptom management, medications, and end‑of‑life care.

You will collaborate with an interdisciplinary team, document care accurately, and participate in on‑call coverage as needed.

Qualifications

• RN with at least 1 year of experience in hospice, home health, or related settings.
• Current nursing license in the state of employment.
• CPR Certification and valid driver's license with auto insurance.

Responsibilities

• Be the primary contact for patients and families guiding them through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home settings.
• Perform initial and ongoing assessments to monitor conditions.
• Collaborate with an interdisciplinary team to build personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
• Maintain accurate clinical documentation and ensure infection control standards.
