Job summary

Crescent Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our caring nursing team delivering end-of-life care across the Conway community. You will provide hands-on nursing, coordinate services, and guide patients and families through the hospice journey.

As part of our interdisciplinary team, you’ll collaborate with the Clinical Director, advocate for patients, educate families, and participate in on-call duties.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.
• CPR certification if state requires.
• Valid driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise; address inquiries compassionately.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct nursing care and support to maintain patient comfort and quality of life.
• Deliver nursing with heart using proven principles and techniques.
• Educate and advocate for patients and families to create personalized care plans.
• Share on-call duties and be there when it matters most.
