Crescent Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) for a Per Diem role in the Columbia area. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through the hospice journey.

Qualifications

• RN license in good standing.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with EMR systems and general computer use; CPR certification if required by state.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with the Clinical Director to assess eligibility and care needs.
• Deliver compassionate care and education to patients and families.
• Engage patients and families to create personalized care plans.
• Participate in call duties as part of the team.
