Job summary

Crescent Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to provide per diem hospice nursing and care coordination across York, Chester, and Lancaster counties. You will work with an interdisciplinary team to deliver hands-on care, guide patients and families, and ensure high-quality, compassionate end-of-life support.

The role emphasizes education, advocacy, and collaboration with the Clinical Director, with a focus on timely, patient-centered care in a community setting.

Qualifications

• Must hold an active RN license and be licensed without encumbrances.
• Minimum 1 year of RN experience in medical-surgical/acute care; hospice experience a plus.
• CPR certification may be required by state and must be maintained.

Responsibilities

• Respond to clinical referral information with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Deliver hands-on hospice nursing care and coordinate services to support patients and families.
• Provide nursing care grounded in proven principles to ease pain and improve quality of life.
• Educate patients and families to participate in personalized care plans.
• Participate in after-hours call duties as part of a compassionate team.
