Job summary

Crescent Hospice in Conway is seeking a compassionate Hospice Registered Nurse Case Manager (RNCM) for a per diem role. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their journey in the community.

The ideal candidate holds an active RN license, with at least 1 year in medical-surgical/acute care; hospice experience is a plus. CPR certification, EMR proficiency, and reliable transportation are required.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.
