Job summary

Crescent Hospice is seeking a Per Diem Registered Nurse Case Manager to join our compassionate nursing team delivering end-of-life care in Greenville. You will coordinate services, support patients and families, and ensure comfort and dignity throughout the hospice journey.

The role offers Monday–Friday 8:00 am–4:30 pm, a community-based territory, and a comprehensive benefits package. Ideal candidates hold an RN license, have 1 year of medical-surgical experience, and are proficient with EMR

Qualifications

Current unencumbered RN license.
Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
Proficient with computers and EMR systems.
CPR certification if required by state.
Driver's license and reliable transportation.

Responsibilities

Respond to referrals with compassionate, expert care.
Collaborate with Clinical Director to assess client eligibility for hospice services.
Provide direct nursing care and coordinate services for patients.
Deliver care with heart to ease pain and improve quality of life.
Educate patients and families to develop personalized care plans.
Participate in on-call duties as part of the team.
