Job summary

Crescent Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our compassionate team serving Union county and surrounding areas in South Carolina. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through their care journey.

The ideal candidate is a motivated RN with at least 1 year in medical-surgical or acute care settings, proficient with EMR, CPR certified, and holds a valid driver's license.

Qualifications

• Must hold current unencumbered RN license.
• Minimum one year experience as an RN in a medical-surgical/acute care setting; hospice experience a plus.
• Must be computer proficient typing and have EMR experience.
• Possess and maintain CPR certification if required by state.
• Have a current driver’s license and reliable transportation.

Responsibilities

• Empower through engagement: respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess eligibility and suitability of clients for hospice services.
• Nurture direct care: coordinate care while ensuring comfort and quality of life.
• Provide nursing with heart: deliver top-notch care rooted in proven principles.
• Educate and advocate: educate patients and families to create personalized care plans.
• Participate in on-call rotations to be there when it matters most.
