Job summary

Crescent Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem coverage in the Conway area. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey with a dedicated interdisciplinary team.

Responsibilities include collaborating with the Clinical Director, educating families, and participating in on-call duties to ensure The Best Care Possible with compassion and clinical

Qualifications

• Must hold current unencumbered RN license.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting; hospice experience a plus.
• Proficient with computers and EMR systems.
• CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Partner with Clinical Director to assess eligibility and suitability of clients for hospice services.
• As an RN Case Manager, coordinate care and provide comfort and support to patients, ensuring quality of life.
• Deliver nursing care rooted in proven principles and techniques to ease pain and provide solace.
• Educate patients and families to create personalized care plans aligned with goals.
• Participate in on-call duties as part of the team.
