Job summary

Four Seasons is seeking an RN Case Manager to coordinate nursing plans of care across Henderson County and surrounding areas. The role involves direct patient care, supervising LPNs and Hospice Aide staff, and ensuring high-quality, compliant nursing services within a Hospice program.

The position supports a salaried, exempt structure with standard weekday hours and flexible weekend/holiday call as needed. A valid NC RN license and driver’s license are required.

Qualifications

• Two years of clinical experience preferred.

Responsibilities

• Coordinate the nursing plan of care and supervise staff.
• Provide direct nursing care to a caseload of patients.
• Assess, plan, implement and evaluate nursing care.
• Educate patients and families about care plans.
• Ensure compliance with regulations and reimbursement rules.
