Job summary

Four Seasons is seeking an RN Case Manager to coordinate nursing plans of care, provide direct nursing care, and supervise LPN and Hospice Aide staff for a caseload of approximately 15 patients in Buncombe County and surrounding areas.

This exempt, salaried position requires strong knowledge of regulations, infection control, and patient-centered care with a focus on collaboration across departments. Monday–Friday, 8:00 AM–4:30 PM with rotating on-call; location varies within the region.

Qualifications

• Two years of clinical experience preferred.
• Valid NC RN license.
• Valid Driver’s License and proof of automobile insurance.

Responsibilities

• Coordinate the nursing plan of care for a caseload of patients.
• Provide direct nursing care and supervise LPNs and Hospice Aides.
• Teach and supervise staff implementing treatment regimens.
• Collaborate with other health care providers on patient plans.
• Adhere to HIPAA and Medicare Conditions of Participation requirements.
• Maintain accurate documentation of care and patient responses.
• Ensure safe and effective nursing care across shifts.
