Job summary

Four Seasons is seeking an RN Case Manager to coordinate nursing plans of care across a ~15 patient caseload in Macon County and surrounding areas. You will supervise LPNs and Hospice Aide staff, deliver direct nursing care, and work with the Clinical Team Leader to ensure high-quality patient outcomes.

The role requires NC RN licensure, strong communication, and proficiency with EMRs. Monday–Friday with some on-call coverage; full-time, part-time, or PRN options may apply.

Qualifications

• Two years of clinical experience preferred.
• Valid NC RN license.
• Valid Driver’s License with proof of insurance.

Responsibilities

• Coordinate nursing care for an assigned caseload.
• Assess patient health status and plan care.
• Teach and supervise LPNs and Hospice Aides; collaborate with providers.
• Maintain accurate documentation of care and responses.
