Job summary

Four Seasons is seeking an RN Case Manager in WNC to coordinate nursing care, supervise LPNs and Hospice Aides, and manage a diverse caseload. You will assess patient needs, develop and implement care plans, and collaborate with the healthcare team to ensure quality outcomes.

The role requires NC RN licensure, strong communication, and the ability to work with interdisciplinary teams. Full-time or part-time options with occasional on-call duties are available.

Qualifications

• Must be willing and able to care for patients with infectious disease processes.
• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
• Knowledge of HIPAA rules and regulations.
• Knowledge of electronic medical records systems.
• Demonstrate excellent written and oral communication skills.
• Ability to work under pressure to meet deadlines.
• Ability to take initiative and utilize innovative techniques.

Responsibilities

• Coordinate nursing plan of care and supervise LPN and Hospice Aide staff.
• Manage nursing care for a caseload of ~15 patients with varying acuity.
• Assess patient health and document results.
• Plan and deliver nursing acts; teach and supervise staff.
• Collaborate with other health care providers for patient plans.
• Implement prescribed regimens per State law.
• Provide patient health teaching and counseling.
• Record care plans and patient responses; supervise staff.
