Job summary

Four Seasons is seeking an RN Case Manager to coordinate nursing care for an approximate caseload of 15 patients with varying acuity. You will supervise LPNs and Hospice Aide staff while delivering direct care and teaching patient families.

Reports to Clinical Team Leader. Requires NC RN license and strong EMR proficiency. Position includes FT/PT/PRN options and rotating on-call duties in Henderson County area.

Qualifications

• Two years of clinical experience preferred.
• Must be licensed as RN in NC.
• Experience with electronic medical records (EMR).
• Strong written and verbal communication skills.
• Ability to collaborate with multiple departments.

Responsibilities

• Coordinates nursing plan of care for an assigned caseload.
• Provides direct nursing care and supervises LPN and Hospice Aide staff.
• Assess patients' health status and prepare/update care plans.
• Ensure compliance with regulations and participate in team care planning.
• Deliver patient and family education regarding health and treatment plans.
