Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead in-home patient care, assessments, and education. You will work with families and an interdisciplinary team to deliver compassionate, evidence-based end-of-life care in patients' homes or setting of choice.

The role emphasizes accurate documentation, regulatory compliance, and ongoing communication with physicians, social workers, and care coordinators to ensure individualized plans of care.

Qualifications

• Minimum 1 year RN experience in hospice, home health, or related fields.
• Hospice or palliative care experience preferred.
• Current nursing license in the state of employment.
• CPR certification and valid driver’s license.

Responsabilités

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver skilled nursing care per Plan of Care in patient homes.
• Coordinate care with an interdisciplinary team.
• Educate patients and caregivers on disease, self-care, and end-of-life options.
• Document care plans and maintain timely records.
• Support families with bereavement guidance and emotional support.
