Job summary

Anchor Health, a premier hospice organization, seeks a compassionate RN Case Manager to coordinate home-based hospice plans, assess patient needs, manage symptoms, and collaborate with an interdisciplinary team.

Requires CA RN license; hospice experience preferred; valid driver’s license and reliable transportation required. Salary up to $60/hr; schedule Monday–Friday; work on the road.

Qualifications

• RN License is required.
• Hospice care experience preferred (1 year).

Responsibilities

• Completes initial and ongoing patient assessments to determine hospice needs.
• Provides professional nursing care using the nursing process and administers prescribed treatments.
• Maintains and updates the care plan; coordinates with physicians and the interdisciplinary team.
• Educates patient and family on care plans, medications, and palliative options.
• Assists patients with activities of daily living and supports family/caregiver needs.
• Acts as Case Manager to coordinate patient care for assigned caseload.
• Documents clinical notes and reports changes in patient condition to the physician.
