Job summary

Anchor Health is seeking a Hospice RN Case Manager to coordinate end-of-life care for patients in their homes and communities. The successful candidate will assess patient needs, develop and revise the care plan, facilitate education for patients and families, and work with an interdisciplinary team to ensure comprehensive, compassionate care.

The role requires CA RN licensure, CPR certification, and a valid driver's license, with hospice experience preferred.

Qualifications

• Registered Nurse with CA state license.
• Hospice experience preferred, 1–2 years recommended.
• Maintains CPR certification and valid driver’s license for community visits.

Responsibilities

• Provides comprehensive hospice care, including pain management and symptom control, education, and family support.
• Performs initial and ongoing assessments, writes and revises the plan of care.
• Coordinates patient care as Case Manager with the interdisciplinary team and physicians.
• Document clinical notes and communicates changes in patient status to physicians and caregivers.
• Facilitates patient and family teaching, support, and end-of-life care planning.
• Travels to patient locations and maintains a safe care environment.
