Job summary

Anchor Health, a premier hospice provider, is seeking a dedicated Hospice RN Case Manager to coordinate home-based care for patients with life-limiting illnesses. You will develop and manage comprehensive care plans, coordinate interdisciplinary support, and ensure compassionate, patient-centered nursing.

The role requires CA RN license, CPR, a valid driver's license, and willingness to travel to patient locations. Hospice experience is preferred.

Qualifications

• Registered Nurse with valid CA state license.
• Hospice experience preferred; at least one year of hospice and strong acute care foundation.
• Maintains a current CPR certification.
• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient locations.

Responsibilities

• Provides care to patients with terminal illness; manages pain and symptoms; updates the plan of care.
• Acts as Case Manager coordinating patient care with the interdisciplinary team.
• Educates patients and families on self-care and care options within home settings.
• Communicates with physicians and community health professionals to coordinate care.
• Attends interdisciplinary group meetings and maintains accurate clinical notes.
