Job summary

ANCHOR HEALTH is seeking a Hospice RN Case Manager to coordinate and direct hospice care for patients in home and community settings. The role emphasizes creating and updating care plans, administering treatments, and collaborating with an interdisciplinary team to support patients and families.

The ideal candidate holds an active California RN license, CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• RN license required; California license preferred not specified but required.
• Hospice care experience preferred, with at least one year of hospice experience and a solid foundation in acute care.
• CPR certification maintained.
• Valid driver’s license, auto insurance, reliable transportation, and willingness to drive to patient locations.
• Excellent interpersonal skills and a passion for Hospice care.

Responsibilities

• Completes an initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Writes and initiates plan of care, regularly re-evaluating patient and family/caregiver needs.
• Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as prescribed.
• Counsels the patient and family in meeting nursing and related needs and provides health instructions as appropriate.
• Maintains accurate and relevant clinical notes regarding patient condition and care given.
