Job summary

Anchor Health is seeking a dedicated Hospice RN Case Manager to coordinate home-based hospice care for patients with terminal illnesses. The RN will assess needs, implement and adjust care plans, administer medications, and educate patients and families, while collaborating with physicians and the interdisciplinary team.

Requires CA RN license and CPR certification; travel is required. Position offers a Monday–Friday schedule with mileage reimbursement and a sign-on bonus; benefits reflect

Qualifications

• Registered Nurse with CA state license.
• Hospice experience preferred (1 year).
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Excellent interpersonal skills and compassion.

Responsibilities

• Provide care to terminally ill patients; review and update care plan.
• Administer medications and manage pain and symptoms.
• Assess patient and family needs; develop and revise care plan.
• Educate patient and family; coordinate with physician and care team.
• Document clinical notes and communications.
