Job summary

Vitality Hospice is seeking a dedicated RN Case Manager in Bucks and Montgomery County, PA. The role is Monday–Friday, 8:00 AM–5:00 PM, with rotating on-call and a ~15-patient caseload.

You will serve as the clinical lead for your assigned patients, conducting comprehensive nursing assessments, developing and managing individualized plans of care, and collaborating with physicians, LPNs, social workers, chaplains and aides.

Qualifications

• Active Pennsylvania RN License.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive nursing assessments with Admissions Nurse support.
• Develop and manage individualized plans of care.
• Collaborate with physicians, LPNs, social workers, chaplains and aides.
• Educate families on symptom management and disease progression.
• Participate in IDT meetings.
• Maintain timely, compliant documentation.

Caseload: Approximately 15 patients (subject to change based on census and acuity)
