Vitality Hospice & Palliative Care is seeking a Registered Nurse Case Manager (RNCM) to join a compassionate interdisciplinary team. The role emphasizes admissions, ongoing assessments, and personalized care plans across the patient's journey in a hospice setting.

Travel to patient residences is required as part of the daily responsibilities; the position is full-time with regular Monday-Friday hours and occasional evenings/weekends.

Qualifications

• Current Registered Nurse (RN) license required.
• Hospice or home health experience preferred.
• Strong communication, organizational, and interpersonal skills.
• Valid driver's license, reliable vehicle, and current auto insurance.
• Ability to travel within the assigned service area.

Responsibilities

• Conduct patient admissions and ongoing nursing assessments.
• Develop and implement individualized plans of care based on each patient's needs and goals.
• Collaborate with hospice physicians and the interdisciplinary team (IDT).
• Supervise and coordinate care with home health aides, social workers, and chaplains.
• Educate patients and caregivers about disease progression, symptom management, and resources.
• Ensure timely and accurate documentation in compliance with hospice standards.
• Promote a culture of excellence, empathy, and accountability in every interaction.
