Job summary

Vitality Hospice & Palliative Care is seeking an RN Case Manager to serve as the clinical lead for a ~15-patient caseload in Bucks and Montgomery County. The role follows a Monday–Friday schedule (8:00 AM–5:00 PM) with rotating on-call and limited evenings/weekends, supported by an Admissions Nurse and on-call staff.

You will assess patients, develop plans of care, coordinate with physicians, LPNs, social workers, chaplains & aides, educate families on symptom management, and participate in IDT

Qualifications

• Active Pennsylvania RN license.
• Hospice or home health experience strongly preferred.
• Valid driver’s license and reliable transportation.
• Strong clinical judgment and communication skills.

Responsibilities

• Conduct comprehensive nursing assessments (with Admissions Nurse support).
• Develop and manage individualized plans of care.
• Collaborate with physicians, LPNs, social workers, chaplains & aides.
• Educate families on symptom management and disease progression.
• Participate in IDT meetings.
• Maintain timely, compliant documentation.
• Support assigned patients as clinical lead with interdisciplinary team.

Location: Bucks and Montgomery County
Schedule: Monday–Friday | 8:00 AM–5:00 PM
On-Call: Rotating | Limited occasional evenings/weekends
Caseload: Approximately 15 patients (subject to change based on census and acuity)
