Job summary

Vitality Hospice and Palliative Care in Pennsylvania is seeking an RN Case Manager to serve as the clinical lead for an assigned patient panel within a weekday schedule. You will collaborate with a multidisciplinary team to implement and monitor individualized care plans, ensuring symptom control and family education.

You will conduct comprehensive assessments, participate in IDT meetings, and maintain accurate documentation while balancing a ~15-patient caseload and on-call responsibilities

Qualifications

• Active Pennsylvania RN license required.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills.
• Valid driver’s license and reliable transportation.

Hospice RN Case Manager – $85,000–$100,000 | Mon–Fri | ~15-Patient Caseload

Location: Bucks and Montgomery County
Schedule: Monday–Friday | 8:00 AM–5:00 PM
On-Call: Rotating | Limited occasional evenings/weekends
Caseload: Approximately 15 patients (subject to change based on census and acuity)

Your Role
