Vitality Hospice & Palliative Care is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our clinician-owned team serving patients and families in Bucks County, PA. This full-time role offers a Monday–Friday schedule with occasional evenings/weekends, a salary range of $85,000–$100,000, and a supportive work environment focused on high‑quality end-of-life care.

Key Responsibilities:

As a Registered Nurse Case Manager, you will play a vital role in our interdisciplinary care team, ensuring that each patient receives personalized, high-quality care.

• Conduct patient admissions and ongoing nursing assessments
• Develop and implement individualized plans of care based on each patient's needs and goals
• Collaborate closely with hospice physicians and the interdisciplinary team (IDT)
• Supervise and coordinate care with home health aides, social workers, and chaplains
• Educate patients and caregivers about disease progression, symptom management, and available resources
• Ensure timely and accurate documentation in compliance with hospice standards
• Promote a culture of excellence, empathy, and accountability in every interaction

Qualifications:

• Current Registered Nurse (RN) License in Pennsylvania (required)
• Clinical nursing experience (hospice or home health preferred)
• Strong communication, organizational, and interpersonal skills
• Valid driver's license, reliable vehicle, and current auto insurance
• Ability to travel within the assigned service area
