Job summary

Vitality Hospice and Palliative Care in Cherry Hill, NJ seeks an RN Case Manager to lead clinical care for a ~15-patient caseload on a Monday–Friday schedule, with rotating on-call and occasional evenings/weekends.

You will conduct comprehensive nursing assessments, develop and manage patient care plans, collaborate with physicians, LPNs, social workers, chaplains and aides, educate families on symptom management, and document diligently.

Qualifications

• Active New Jersey RN License required.
• Hospice or home health experience preferred.
• Strong clinical judgment and communication skills required.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Conduct comprehensive nursing assessments (with Admissions Nurse support).
• Develop and manage individualized plans of care.
• Collaborate with physicians, LPNs, social workers, chaplains & aides.
• Educate families on symptom management and disease progression.
• Participate in IDT meetings.
• Maintain timely, compliant documentation.

Location: Cherry Hill NJ
Schedule: Monday–Friday | 8:00 AM–5:00 PM
On-Call: Rotating | Limited occasional evenings/weekends
Caseload: Approximately 15 patients (subject to change based on census and acuity)

Your Role

As an RN Case Manager, you are the clinical lead for your assigned patients — supported by a strong interdisciplinary team.
