Job summary

Vitality Hospice & Palliative Care is seeking a Registered Nurse Case Manager (RNCM) in Pennsylvania to join our compassionate team. This full-time role covers admissions, care planning, and coordination with the interdisciplinary team to deliver high-quality hospice care.

You will travel within the service area, maintain timely documentation, and educate patients and families. Licensed PA RN with hospice or home health experience preferred; schedule is Monday–Friday, 8:00 AM–5:00 PM with

Key Responsibilities:

As a Registered Nurse Case Manager, you will play a vital role in our interdisciplinary care team, ensuring that each patient receives personalized, high-quality care.

• Conduct patient admissions and ongoing nursing assessments

• Ensure timely and accurate documentation in compliance with hospice standards
• Promote a culture of excellence, empathy, and accountability in every interaction

Qualifications:

• Current Registered Nurse (RN) License in Pennsylvania (required)
• Clinical nursing experience (hospice or home health preferred)
• Strong communication, organizational, and interpersonal skills
• Valid driver’s license, reliable vehicle, and current auto insurance
• Ability to travel within the assigned service area
