Job summary

Resource 1 Staffing is seeking a Hospice Registered Nurse (RN) Case Manager to join a growing hospice organization serving the Greater Philadelphia region. This full-time position covers Philadelphia, Bucks, Chester, Delaware, and Montgomery counties, with a Monday–Friday schedule and on-call rotation about every three weeks.

The role emphasizes compassionate, patient-centered care across home and facility settings, with caseloads of roughly 14–16 patients and 4–5 visits daily.

Qualifications

• Active RN license.
• At least 1 year of clinical experience (acute, home health or hospice preferred).
• Valid driver’s license and reliable transportation.

Responsibilities

• Manage hospice patient caseload across home and facility settings.
• Perform patient assessments and develop individualized care plans.
• Provide skilled nursing care and symptom management.
• Coordinate with interdisciplinary team members.
• Maintain accurate and timely clinical documentation.
• Document in WellSky EMR.
• Support patients and families throughout the hospice journey.

This organization is known for delivering compassionate, patient-centered care through interdisciplinary teams in both home and facility settings while maintaining a supportive, team-oriented clinical environment.

We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).

Pay and Schedule

$85,000–$105,000 annually based on experience

• Monday–Friday schedule w/ some on-call rotation approximately every 3 weeks (rare to get a call)
• Flexible scheduling structure with leadership support

• Patients are assigned geographically to support efficiency
• Mix of facility and home-based hospice care
• Travel throughout the territory is required
