Vitality Hospice is seeking a Registered Nurse Case Manager (RNCM) in Pennsylvania to join a compassionate interdisciplinary team dedicated to end-of-life care.

This full-time role requires an active PA RN license, strong communication skills, and the ability to travel within the service area. You will lead admissions, care planning, and collaborate with IDT while educating families and ensuring quality documentation.

Qualifications

• Current RN license in Pennsylvania is required.
• Hospice or home health experience preferred.
• Valid driver’s license and reliable vehicle.
• Ability to travel within the assigned service area.
• Strong communication and organizational skills.

Responsibilities

• Conduct patient admissions and ongoing nursing assessments.
• Develop and implement individualized care plans based on patient needs and goals.
• Collaborate with hospice physicians and the interdisciplinary team (IDT).
• Supervise and coordinate care with home health aides, social workers, and chaplains.
• Educate patients and caregivers about disease progression, symptom management, and resources.
• Ensure timely and accurate documentation in compliance with hospice standards.
• Promote a culture of excellence, empathy, and accountability.
