Job summary

Ocean Healthcare is seeking a compassionate Registered Nurse (RN) Hospice Case Manager in Monroe County, PA to deliver patient-centered end-of-life care in patients’ homes and local settings.

You will perform assessments, develop care plans, and collaborate with physicians and social workers while educating families and ensuring timely documentation. There are no nights or weekends, mileage reimbursement, and robust benefits.

Qualifications

• PA RN license required.
• Minimum 1 year of nursing experience (hospice, home health, or case management preferred).
• Strong clinical, organizational, and communication skills.
• Compassionate, patient-focused approach to care.
• Valid driver’s license with reliable transportation.

Responsibilities

• Manage and coordinate care for an assigned hospice caseload.
• Perform comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing visits in patients’ homes and other care settings.
• Collaborate with physicians, social workers, chaplains, aides, and other team members.
• Educate patients and families on disease progression, symptom management, and medications.
• Monitor patient status and adjust care plans as needed.
• Ensure accurate, timely clinical documentation in accordance with hospice regulations.
• Participate in interdisciplinary team meetings and care planning.
• Serve as a patient and family advocate throughout the hospice journey.

• We are seeking a compassionate and experienced Registered Nurse (RN) Hospice Case Manager to join our interdisciplinary hospice team serving the Monroe County area. This role focuses on providing high-quality, patient-centered care to individuals and their families during end-of-life, while supporting comfort, dignity, and quality of life.
