Job summary

Grace Hospice in Cincinnati seeks an RN Case Manager to deliver end-of-life care in patients' homes and communities. You will coordinate nursing services, supervise aides, and partner with physicians to develop compassionate care plans.

This full-time role requires strong clinical judgment, excellent communication, and the ability to support patients and families through sensitive, meaningful experiences; on-call and weekday schedule are included.

Qualifications

  • Current unrestricted RN licensure in the state of practice.
  • Valid driver’s license and proof of auto insurance.
  • Two years of RN experience.
  • Two years of Hospice/Palliative Nursing experience preferred.

Responsibilities

  • Assume primary responsibility for the patient/family caseload during on-call hours in collaboration with the triage team.
  • Coordinate care plans and ensure timely visit notes and documentation.
  • Assist, provide, or arrange medications and supplies after hours, weekends, and holidays.
  • Attend interdisciplinary team meetings during on-call periods for patient/family needs evaluation.
  • Admit patients eligible for hospice services under physician orders.
  • Communicate patient status and needs to leadership and clinicians.
  • Develop care plans addressing identified problems with mutually agreed goals.
  • Apply 4Ms framework and Age-Friendly Care in patient interactions.

The RN Case Manager is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care.

This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences.

How you will make an impact:
  • Assume primary responsibility for the patient/family caseload that includes assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team
  • Report On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift
  • Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information
  • Assist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidays
  • As scheduled, attend interdisciplinary team meetings during on-call period for full team discussion, consultation and evaluation of patient/family needs
  • Under the physician’s order, admits patients eligible for hospice services in collaboration with the triage team
  • Reports patient status and need for other disciplines to clinical leadership, attending physician and hospice physician
  • Develops patient care plan that specifically addresses identified patient problems; nursing problems and goals that has been identified as mutually agreed upon goals with patient(s) during on-call hours, weekends and/or holiday(s)
  • Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

Schedule: Mon-Fri 8am-5pm. On call as needed.
