Job summary

Gentiva is seeking a compassionate Hospice Registered Nurse to provide hands-on clinical care and case management in patients’ homes or in related settings across Oklahoma City. You will coordinate with physicians, LPNs, CNAs, social workers, and chaplains to develop and implement individualized plans of care and educate families on disease management and comfort measures.

Qualifications

• Current license to practice nursing in the state of requested employment.
• CPR Certification.
• Current automobile insurance and valid driver’s license.
• Minimum of one year RN experience in hospice, home health, or related settings; hospice/palliative care preferred.

Responsibilities

• Be the primary contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on each patient’s Plan of Care in home or facility settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
