Job summary

Gentiva Hospice is seeking an experienced Registered Nurse Case Manager to lead in-home and facility-based hospice care. You will act as the primary contact for patients and families, delivering skilled nursing care and coordinating with an interdisciplinary team to build personalized plans of care.

You will perform ongoing assessments, educate caregivers, ensure accurate documentation, and participate in on-call coverage.

Qualifications

• Must be a licensed Registered Nurse with current state license.
• Minimum 1 year RN experience in acute care, hospice, oncology, or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers about disease, end-of-life care, and medications.
• Document clinical records timely and maintain quality documentation.
• Support on-call and patient support activities as applicable.
