Gentiva is seeking a Hospice RN / RN Case Manager to lead care for patients in home settings, guiding families through the hospice journey with dignity and compassion. This role emphasizes education, bereavement support, and timely documentation. Active RN license, CPR, and at least 1 year in hospice or home health are required; competitive pay and comprehensive

Qualifications

• Active RN license in state of employment
• 1 year minimum RN experience; hospice/palliative care preferred
• Current CPR certification
• Driver’s license and auto insurance

Responsibilities

• Be the primary contact for patients and families guiding hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home or equivalent setting.
• Perform comprehensive and ongoing patient assessments to monitor condition.
• Collaborate with an interdisciplinary team to develop and refine care plans.
• Provide emotional, physical, and spiritual support to patients and families; educate caregivers.
• Educate caregivers and provide practical and emotional training.
• Communicate updates in patient condition to the care team for timely interventions.
• Maintain accurate clinical documentation and comply with infection control standards.
• Support quality improvement and scheduling initiatives to ensure consistent, compassionate care.
• Assist patients and families in understanding hospice services, medications, and end-of-life processes
