Qualifications

• RN license in required state.
• CPR certification required.
• Current automobile insurance and valid driver's license.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver and document skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with appropriate visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients and caregivers on disease, end-of-life care, medications, nutrition, and self-care.
