Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient and family care in home settings. You will assess, plan and deliver skilled nursing care per individualized plans of care, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, comprehensive end-of-life support.

You will educate families, monitor patient conditions, manage symptoms, and document care precisely while adhering to infection control and regulatory standards.

Qualifications

• RN with current license to practice nursing.
• Minimum 1 year RN experience in acute care, hospice, or home health.
• Hospice or palliative care experience preferred.
• CPR certification; valid driver''s license and auto insurance.
• Strong assessment, documentation and regulatory knowledge.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person and telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians and other IDG members.
• Educate patients and caregivers on disease, end-of-life care and medications.
• Document timely clinical records and maintain infection control standards.
