Job summary

Heartland Hospice is seeking an experienced Registered Nurse to serve as an RN Case Manager in home-based hospice care. You will lead patient assessments, develop plans of care, and collaborate with an interdisciplinary team to ensure compassionate, quality end-of-life care.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year experience as an RN in hospice, palliative care, home health, or acute care.

Responsibilities

• Perform comprehensive RN assessments and education for patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update plans of care with visit frequency and risk identification.
• Coordinate care with IDG and physicians; educate on disease, end-of-life care.
• Maintain timely clinical records and ensure compliance with regulatory standards.
• Participate in on-call and patient support activities as applicable.
