Job summary

Heartland Hospice is seeking a dedicated Registered Nurse to join our team as an RN Case Manager. You will lead in-home hospice care, assess patients, and collaborate with an interdisciplinary team to develop and implement plans of care that address medical, emotional, and spiritual needs.

Experience in hospice or home health is preferred, and a valid RN license is required. You will provide education to families, coordinate with physicians, and ensure accurate documentation and compliance.

Qualifications

• RN license in the state of employment
• Minimum 1 year RN experience, hospice or home health preferred
• Hospice or palliative care experience is a plus

Responsibilities

• Perform comprehensive patient assessments and reassessments and provide education to patients, families and caregivers.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients and caregivers about disease, self-care, medications, nutrition and end-of-life care.
• Communicate changes in condition to the care team for timely interventions.
• Support continuity of care and transitions between home health, palliative and hospice levels.
• Document timely clinical records including orders, progress notes and care plans.
